Thank you for agreeing to complete this short survey to help us understand your attitude and behaviours towards being physically active.
Physical activity (any activity that raises heart and breathing rate) is good for people’s health and wellbeing and we would like to ask you some questions about your physical activity habits. Your answers will help inform our new Active Wellbeing Strategy and help us understand what barriers may prevent you from being more physically active and what we can do to help overcome these and encourage you to lead a more healthy and active life. Your responses are completely anonymous.

Moderate intensity activity will raise your heart rate, make you breathe faster and feel warmer than normal (e.g. brisk walking, cycling).
